Glaucoma Surgery
 

Glaucoma

Glaucoma is a group of diseases affecting the optic nerve that results in vision loss and is frequently characterized by raised intraocular pressure (IOP). There are many glaucoma surgeries, and variations or combinations of those surgeries, that facilitate the escape of excess aqueous humor from the eye to lower intraocular pressure, and a few that lower IOP by decreasing the production of aqueous.

At ECC, Dr. Pierre Mardelli, a fellowship-trained glaucoma expert, specializes in this type of surgeries:

Argon Laser Trabeculectomy (ALT)

In some selected patients whose IOP is borderline, the argon laser beam can be used to apply burns to areas of the trabecular meshwork, located near the base of the iris, to increase fluid outflow. ALT is used in the treatment of various open-angle glaucomas to safely lower the intraocular pressure. The procedure is done in the center and the patient can leave home un-assisted.


 

Iridotomy

An iridotomy involves making puncture-like openings through the iris without the removal of iris tissue. A Nd:YAG laser is used to decrease the IOP in patients with angle-closure glaucoma. A laser peripheral iridotomy (LPI) is the application of a laser beam to selectively burn a hole through the iris near its base. LPI may be performed with either an argon laser or Nd:YAG laser. This procedure is quick and painless.  This procedure can be sight-saving in the setting of a narrow angle or in cases of sub-acute angle closure glaucoma.

Filtering Procedures

Filtering surgeries are the mainstay of surgical treatment to control intraocular pressure. An anterior sclerotomy or sclerostomy is used to gain access to the inner layers of the eye in order to create a drainage channel from the anterior chamber to the external surface of the eye under the conjunctiva, allowing aqueous to seep into a bleb from which it is slowly absorbed. 

Trabeculectomy

Trabeculectomy is a guarded filtering procedure that removes of part of the trabecular meshwork. Dr. Mardelli is a leader in this field and has introduced multiple new techniques to advance this type of surgery like the Flap-lift suture (FLS), the modified Wise closure suture, and the needle revision of the failed blebs using Mitomycin -C at the slit-lamp.

Other surgical procedures

Goniotomy and trabeculotomy are two procedures mostly used in the treatment of congenital glaucoma in infants and children.  They both involve microsurgical dissection with mechanical disruption of the trabecular meshwork.

Tube-shunt surgery or drainage implant surgery

This involves the placement of a tube or glaucoma valves to facilitate aqueous outflow from the anterior chamber.  Dr. Mardelli has a vast experience with this type of surgery and has presented a large case series illustrating a novel technique in the placement of these devices.
